The Program Manager will report directly to the Director of Health Services and is responsible for the daily management of the Palliative Care program at George's House. The Program Manager adheres to ethical standards in all activities relating to this job description and works within the context of a client-centered care model supporting the vision and values of the George Spady Society.

The George's House Program Manager is responsible for managing a team of Licensed Practical Nurses (LPNs) and Health Care Aides (HCAs) who provide palliative end of life care within a trauma informed framework. The Manager is also responsible for connecting clients to appropriate community services and resources to support their care. In addition to oversight responsibilities, the Program Manager works on the frontline alongside staff to ensure clients are admitted, supported, and connected to the resources they need. The Program Manager will provide leadership, consultation, training, and support to staff in their work with clients and is responsible for overseeing daily adherence to policies and procedures in program delivery. The Program Manager will work with staff to ensure the Palliative Care Program at George's House provides a safe, communal living environment for persons who are structurally vulnerable and need palliative end-of-life care and mental health supports. This includes clients who may be actively using substances.

Description of Duties and Responsibilities

-Works with DES to ensure staff are compliant with Accreditation Standards, and are following best practice and safety protocols
-Prepare monthly program report, comply with data and information collection and reporting requirements for clients, staff, program delivery, and contractual obligations
-Ensure that clients are being served within a client-centered and trauma informed approach, and that service models are being practiced accurately
-Facilitate intakes and admissions, and complete comprehensive psychosocial assessments for all clients who are referred to treatment
-Work directly with clients, families, and natural supports to ensure clients feel welcomed, informed, and supported throughout their stay
-Collect, sort, and maintain up-to-date information on resources and services available, and assist clients to access these services
-Provide ongoing end-of-life support to clients and their chosen families
-Adhere to the George Spady Society code of ethics
-Update and maintain information on referral sources using the FYI database
-Attend case conferences and interagency meetings in the community as time allows
-Facilitate referrals for the clients in George's House to programs as needed, i.e., AISH
-Work alongside the LPN and HCA team to see that client care goals are being met and that clients have what they need day to day
-Participate in weekly rounds with care team
-Provide emotional and psychological support when needed
-Support staff and step in directly, as needed, in the warm hand off of clients to medical, wellness, and recovery appointment
-Provide necessary support during crises, emergencies, and end of life care
-Interact with clients in a non-judgmental way, using de-escalation and rapport building techniques
-Co-ordinate with interdisciplinary teams such as physicians, AHS staff, case managers, home care workers, etc.
-Engage with families and natural supports to keep them informed and involved in the client's care where appropriate
-Participate in team efforts to maintain a healthy physical environment for our clients
-Complete a variety of administrative, managerial, and other related duties for operations, client-centered care, and service delivery of the palliative care program at George's House in collaboration with the staff and DES
-Oversee all aspects of staff management: manage shift schedules, approve staff leaves and vacation requests, ensure training compliance, complete staff evaluations, encourage training and development, and oversee recruitment and hiring selection

Position Qualifications

-University or College degree in human services or related field
-Minimum 2 years' experience working in the addictions and/or palliative fields
-Minimum 2 years' experience of direct program and staff management
-Understanding of inner-city issues and addictions field
-Knowledge of referral resources
-Experience working with people with complex life circumstances associated with substance-related disorders, mental health issues, mobility issues and homelessness
-Basic understanding of care principles based on dignity, hope and healing as individuals move forward towards improved health and well-being
-Ability to work within a multidisciplinary team and manage staff in the delivery of a variety of diverse care tasks for the palliative care program
-An open, accepting, and non-judgmental attitude with an ability to build rapport with clients and staff
-Compassionate and able to handle stress
-2+ Years' experience working with vulnerable populations and the social services sector
-2+ Years' experience working with people in crisis, with acute or chronic mental and/or physical health issues and who may be actively using substances
-Ability to build professional relationships with service users, team members, management, and stakeholders
-Excellent communication and interpersonal skills
-Proficient in MS Outlook, Word, Excel, Databases, and cloud-based software
-Current Vulnerable Sector Police Information Check
-Valid Driver's License
-First Aid/CPR/AED Training

Hours of work: 7.5 hours per day, Monday to Friday, with some after-hours availability required to support staff and clients.
